We understand that sometimes things go missing especially vaccination certificates.

If you have been to one of our clinics and require a replacement certificate, then we will need the following details to verify and then once payment has been made supply the certificate.

  1. Full Name as it appears in your passport (or the name supplied when you attended the clinic)
  2. Date of Birth
  3. Your Address
  4. Date you attended the Clinic
  5. Location/Venue of the Clinic you attended
  6. Reference Number

Please note: Be certain you attended the CBHUK clinic and not one of our partner events.  
Any payments made is to cover administration costs and as such cannot be refunded. 
Vaccination certificates will be posted out 2nd class post.  You will have an option to upgrade the services as required.
Where an electronic copy is required due to time limits, then a charge of no more than £15 will be made per certificate.
